Transition services is defined as a coordinated set of activities that are designed to be within a results-oriented process. The process must be focused on: academic and functional achievement; movement from school to post-school activities; and postsecondary education, vocational education, employment, life long learning, adult services, independent living, and community participation.
Without academic preparation, specific activities to enroll in postsecondary education, and/or specific efforts to obtain a job without a future, children will graduate from high school unprepared and without a plan.
